Shaker Heights is one of only two cities in Cuyahoga County with its own Health Department.

The department:

• Works closely with the Shaker public schools
• Conducts sanitation inspections
• Provides immunizations and diagnostic testing

Public Records: If you would like to make a request for a public record that may be in the Health Department, or if you have a question about the City's public records policy or its implementation, please visit the Public Records Request page or contact the Department's Public Records Manager Sandi Hurley at 216-491-1480 or through email.

Vital Statistics: The City of Shaker Heights Health Department has transferred the function of its Vital Statictics Operation. Burial permits, and certified copies of birth and death certificates are no longer available.

Shaker residents who need certified copies of birth or death records should contact the City of Cleveland Vital Statistics Office at 601 Lakeside Avenue, Cleveland, Ohio 44114 or phone 216-664-2317.

Mr. Robert L. Forward of Brown Forward Funeral Home is now a sub-registrar and can approve burial/cremation permits for funeral homes. His contact information is 216-751-1200.
