Commemorative Bench Donation Application

  1. Thank you for your interest in donating a commemorative bench to be installed in a City of Shaker Heights park. Please complete this application to start the donation process.

    Note: It is important that applicants understand the guidelines for the donation program before proceeding. To this end, the City asks applicants to review the Commemorative Bench & Plaque Donation and Maintenance Agreement (PDF) before submitting this application.

    The City will not accept applications from individuals who have not reviewed the Commemorative Bench & Plaque Donation and Maintenance Agreement (PDF). By checking Yes, you are acknowledging that you have reviewed the document.
  3. Please note: Inscriptions should be between 25 and 100 characters; Recreation Dept. staff will work with the applicant to finalize the inscription.

    Your bench donation request will be reviewed by City staff. Once approved, the donor will be asked to sign a Commemorative Bench & Plaque Donation and Maintenance Agreement and submit payment.
